Grooming Consent Form

I understand that a current rabies and distemper (and Bordetella for dogs) are required upon admission into Kittery Animal Hospital.

I also understand that a current (within a calendar year) heartworm test and stool check for intestinal parasites must also be up to date.

SEVERE MATTING: Matted coats can cause a variety of skin and health problems. Matted fur does not allow the air to circulate to the skim, which can increase the risk of bacteria, infection, and parasites. Mats that are left in the coat will tighten close to the skin, and attempting to brush these out can cause pain, discomfort, and stress to your animal. Kittery Animal Hospital wants to do what is best for your pet's health. Heavy matting will require the coat to be shaved short. In the case of matting, a matting fee of $55 for 30 minutes of de-matting will be charged to your bill. I hereby grant permission to the Kittery Animal Hospital groomer on staff to shave matted areas if necessary.

I understand that in the event fleas are noted on my pet, Kittery Animal Hospital will treat with appropriate parasite control medication while my pet is in the hospital, and I will be charged for the medication.
